Flex Programs are requiring submission to the NHSN Annual Facility Survey as it relates to antibiotic stewardship. The survey is typically open in January of each year and the CDC encourages CAHs to have this completed by March 1.

Note: Newly enrolled facilities will be asked to complete a survey upon enrollment.

We recommend printing and completing the paper form then going to enter. Many sections may need consultation with others in the facility and form entry does not allow you to save partial submissions.
